| Abstrak | : | The construct of engagement provides a holistic lens for understanding how children interact with learning activities, with distinct behavioral, emotional−affective, and cognitive components forming a multidimensional engagement profile for each child. As the understanding of engagement and recognition of its complexity grow, a pressing need has emerged for a synthetic, coherent review that simultaneously integrates extant literature and clarifies the conceptualization of engagement, identifies its key facilitators and consequences, and proffers a theoretical framework that elaborates on how engagement functions. Using a developmental−contextual approach, this article integrates empirical and theoretical scholarship to illustrate how engagement is produced by developmental and relational processes involving transactions across multiple ecologies. The integrative model of engagement offers a comprehensive perspective on the multiple pathways—psychological, cognitive, social, and cultural—underlying the development of children’s engagement. Conceptualizing engagement as a multidimensional construct shaped by interactions between an individual and the environment enriches the field’s understanding of the personal, contextual, and sociocultural factors that foster or undermine engagement. | Abstrak | : | Hypoxic spells are serious and potentially fatal complications of cyanotic congenital heart disease. These events typically occur in children and are extremely rare in adults. They are caused by a sudden reduction in pulmonary blood flow, leading to severe hypoxia, cyanosis, seizures, and even death. Although rare, hypoxic spells should be considered as a differential diagnosis in adult patients presenting with hypercyanotic crises, particularly those without a known history of congenital heart disease. Case: A 20-year-old woman presented with worsening shortness of breath and cyanosis over the past two hours. She had a lifelong history of recurrent cyanosis, a habit of squatting during episodes of breathlessness, and limited physical activity but had never been diagnosed with congenital heart disease. Electrocardiography revealed right axis deviation and right ventricular hypertrophy, while a chest X-ray showed cardiomegaly with a boot-shaped heart silhouette. A hypoxia test supported the suspicion that her hypoxia was cardiac in origin. Initial management included the knee-chest position, highflow oxygen therapy, intravenous fluids, morphine, and norepinephrine to increase systemic vascular resistance. Subsequent echocardiography confirmed pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries (PA + VSD + MAPCAs), a rare variant of tetralogy of Fallot. | Abstrak | : | A growing amount of psychological theory and evidence explains pathways between a young person’s socioeconomic background, their identity, and their achievement of academic and career goals. These models provide an important foundation to investigating life trajectories, which can be expanded in 3 specific ways. First, studies can explicitly consider the important role of other social factors that intersect and overlap with socioeconomic considerations, including those related to the experience of race-ethnicity and racism. They can also be expanded to more directly acknowledge the strengths and assets of students from nondominant groups. Last, more research can holistically investigate the connections between achievement goal pursuit and physical health. The current article highlights select empirical studies advancing the psychological study of socioeconomic opportunity in these ways. The article also includes implications for the study of identity and the development of not only interventions but also a reimagining of systemic and institutional support particularly for people who face multiple dimensions of barriers in pursuing opportunities. | Abstrak | : | Tracheostomy is a common procedure in intensive care units for critically ill patients with mechanical ventilation. Case: This case report describes the use of real-time ultrasonography (USG) guidance for percutaneous dilatational tracheostomy (PDT) in a 63-year-old female with prolonged mechanical ventilation following craniotomy. The patient had relative contraindications including short neck and morbid obesity (BMI 40.8). On day 7 of ICU admission, a PDT was successfully performed using real-time ultrasonography (USG) guidance. USG was utilized to identify key anatomical landmarks, such as tracheal rings and vascular structures, ensuring safe and accurate needle placement. The use of real-time USG significantly reduced the risk of complications including hemorrhage, tracheal injury, or pneumothorax. The procedure was completed without incident, and the patient showed clinical improvement post-intervention, including enhanced respiratory function and gradual weaning from mechanical ventilation. Conclusion: This report highlights the safety and efficacy of USG-guided PDT, especially in high-risk patients with difficult neck anatomy. It also underlines the importance of ultrasound as a widely available and cost-effective tool in ICU settings. The case supports further implementation of real-time USG-guided techniques in percutaneous tracheostomy to improve procedural success and patient outcomes. Further research involving a larger cohort is needed to establish standardized protocols and evaluate long-term outcomes of this approach. |
