Benefits of Influenza Vaccination in Children with Neurological or Neurodevelopmental Disorders

Abstrak : Children with neurological or neurodevelopmental disorders (NDDs) are at higher risk for severe flu complications due to limited mobility, weaker respiratory muscles, and abnormal muscle tone, which can affect lung secretion function. Although this group is relatively small, children with NDDs account for a significant number of influenza-related hospitalizations and deaths. Annual vaccination is an effective prevention measure and is recommended for all children aged 6 months to 18 years, including those with NDDs. Family members or caregivers in close contact with these children should also receive annual influenza vaccinations. However, there is still limited research on the influenza vaccine in this group, and its effectiveness can vary each year. This article reviews influenza vaccination in general and specifically for children with NDDs.

Predicting Influenza Virus Mutational Complexity in the Development of Effective Vaccines for Children

Abstrak : Influenza virus infection is common in populations of all ages. Vaccination is the most effective measure to prevent infection. However, influenza viruses are prone to antigenic drift, causing the virus to mutate within a few months. Predicting influenza virus evolution plays a crucial role in ensuring the protective effect of vaccines, allowing for the selection of the appropriate vaccine type. Stacking models, convolutional neural network (CNN) models, Gaussian processes vector autoregressive models, and susceptible-exposed-infectious-removed (SEIR) models can predict influenza virus antigenic variants. Various modern models and approaches, such as influenza antigenic variants (IAV)-CNN models, sequence-based antigenic distance approach (SBA), and ensemble of nonlinear regression models, have been used to improve the efficacy and relevance of vaccination strategies.

Abstrak : Influenza viruses have posed a global threat since the 19th century, presenting serious challenges and threats to public health worldwide. Influenza viruses are among the major seasonal outbreaks impacting health and the economy yearly. Numerous new strains emerge over time due to the antigenic drift of influenza viruses, which undergo mutations. Therefore, vaccines and medical technologies continuously need innovative approaches to stimulate immune responses effectively. mRNA-based influenza vaccines are potentially more effective because of the inclusion of a broader range of antigens that can enhance cellular immunity or expand protection beyond just hemagglutinin (HA) and neuraminidase (NA), they can incorporate more than four antigens (HA) in a single formulation, offering advantages in effectiveness, safety, and rapid large-scale production. mRNA-based influenza vaccines hold significant potential and are expected to benefit children due to their various advantages.

From CLE to SLE: A Case Report from Local Hospital

Abstrak :  Lupus erythematosus (LE) is a combination of various interrelating autoimmune clinical diseases. Cutaneous lupus erythematosus (CLE) is a spectrum of typical autoimmune skin features of systemic lupus erythematosus (SLE). A good recognition of the typical appearance of this skin lesion (CLE) enables diagnosis and good management of SLE. Case: A 26-year-old female with SLE with acute skin lessions (ACLE) skin lesions. Suspicion of SLE became stronger when the antinuclear antibody (ANA) titer reached 1:3,200. The main therapy is methylprednisolone injection followed by a gradual dose reduction. The patient was treated for 6 days in hospital and all lesions improved. Conclusion: The patient in this case had SLE with 6 clinical domains in the EULAR/ACR 2019 criteria and a total score of 34 (>10). Earlier and more accurate diagnosis of SLE can make therapy more effective, especially if CLE is already present.

Lymphoid Neoplasia Resembling Mesothelioma

Abstrak :  Lymphoid neoplasms are abnormalities of the hematopoiesis process of the lymphoid line, which can lead to lymphoma or leukemia. Mesothelioma is an abnormal mesothelial cell growth strongly associated with asbestos exposure. In radiology, mesothelioma is usually described as a tissue-dense lesion with well-defined, spiculated borders attached to the thoracic wall. The picture actually only depicts a tissue attached to the thorax, which can be suspected as mesothelioma, but can also be due to other disease processes, such as leukemia, lymphoma, and tuberculosis. Case: A 58-year-old man presented to the emergency room with complaints of shortness of breath. The patient was initially suspected of mesothelioma with suspicion of blood malignancy, but it turned out that the biopsy results were more likely to lead to infiltration of lymphoid neoplasms. Conclusion: In this patient, a lymphatic neoplasm infiltrated the mesothelial tissue and produced a well-demarcated and spiculated tissue picture resembling mesothelioma. Biopsy examination as the gold standard cannot be replaced by radiologic imaging at this time, as many other disease processes can mimic a malignancy.

Initial Management of Paroxysmal Supraventricular Tachycardia with ST-Segment Depression in the Primary Care Setting

Abstrak : Supraventricular tachycardia (SVT) is a common cardiac emergency that can be life-threatening. Symptoms may include palpitations, chest pain, and difficulty breathing. Case: A 50-year-old male with sudden palpitations since one hour ago, accompanied by chest pain and breathing difficulty. A 12-lead ECG revealed an SVT rhythm with ST-segment depression in leads II, III, aVF, and V2-V6. After initial interventions, which included carotid massage, Valsalva maneuver, and oral bisoprolol, the rhythm converted to sinus rhythm within an hour. ST-segment depression may occur during SVT, which warrants evaluation for ischemic etiology. Conclusion: The report focuses on the initial management of SVT in a primary care setting with constraints on equipment and referral facilities. It's important to note that changes in the ST segment in SVT do not always indicate myocardial infarction, and further examination is necessary to confirm the diagnosis.

Early Detection and Prevention of Suicide Risk in Adolescents

Abstrak : Suicide cases in adolescents are a concern to the World Health Organization (WHO) for immediate early detection, prevention, and management. Medical personnel should be more aware of risk factors, signs, and symptoms, and provide solutions. There are internal and external factors that underlie suicidal behavior. The level of suicidal behavior varies from person to person, early detection is needed. Early detection can be done by using screening tools that assess suicidal thoughts and attempts. The important role of the family as the first place to grow is also very much needed. The family will be a support system in the prevention of suicidal behavior. Cross-sectoral collaboration was needed to reduce the incidence of suicide in adolescents. Health workers need to have comprehensive knowledge and understanding of suicidal behavior. Suicidal behavior disorders need to be identified immediately to determine interventions that are appropriate to the patient’s condition. pharmacological and pharmacological therapies are needed to prevent suicidal behavior. Follow-up after therapy needs to be considered in terms of preventing recurrent suicidal behavior.

Mental Health Status of Pregnant Women in Antenatal Care (ANC) Activities at UPTD Puskesmas Manggar Balikpapan, September-October 2023

Abstrak : Maternal mental health is an important yet often overlooked issue, both globally and nationally. About 10% of pregnant women experience mental health problems during pregnancy, with prevalence rates tending to be higher in developing countries. Methods: This descriptive study involved 100 pregnant women who attended routine antenatal care at UPTD Puskesmas Manggar between September and October 2023. The study aimed todetermine the prevalence and characteristics of maternal mental health status. Data were collected through interviews and the Self Report Questionnaire-20 (SRQ-20). Results: A total of 24% pregnant women were found to have common mental disorders, with fatigue being the most frequently reported symptom. The majority of those affected was multiparous (58.3%), in the third trimester of pregnancy (58.3%) and had no history of abortion (91.7%). Conclusion: The relatively high prevalence of common mental disorders during pregnancy underscores the importance of implementing routine mental health screening as an integral part of antenatal care at the primary healthcare level.

The Relationship between Academic Stress and Parental Social Support with Academic Achievement of Students of the Faculty of Medicine, Tanjungpura University Class of 2020

Abstrak : First-year medical faculty students are one of the populations that often experience academic stress. Academic stress can affect academic achievement. Parents’ social support will make their children able to undergo lectures and show good academic achievement. This research aims to determine the relationship between academic stress and parents’ social support on academic achievement among Medicine Faculty students, Tanjungpura University batch 2020. Methods: A cross-sectional observational analytical study. There search population was Medicine Faculty students, Tanjungpura University batch 2020. The research sample was 239 people, chosen by simple random sampling technique. Data was obtained from filling in personal data, student academic stress scale (SASS) questionnaire, parents’ social support questionnaire, and cumulative grade point average. Bivariate analysis used the Spearman correlation test. Results: The result of the correlation test for academic stress and academic achievement was p=0,146; r=0,094; and the correlation test for parents’ social support and academic achievement was p=0,016; r=0,156. Conclusion: There is an insignificant positive relationship between academic stress and academic achievement and a significant positive relationship between parents’ social support and academic achievement.

Relationship between Empathy Level and Burnout Level of Medical Students during Online Learning at the Faculty of Medicine, Tanjungpura University

Abstrak : Distance or online learning conducted during the COVID-19 pandemic causes a lack of interaction among students which affects the level of empathy of these students. Online learning also causes stress in medical students which can lead to burnout. The purpose of this study was to determine the relationship between the level of empathy and the level of burnout among students of the Tanjungpura University Medical Study Program class of 2019. Methods: The method used in this research is an observational analytic study with a cross-sectional approach. The research population was students of the Tanjungpura University Medical Study Program class of 2019 who were taken using a sampling technique in the form of total sampling with a minimum sample size of 76 people. Bivariate analysis using Spearman correlation test. Results: The results obtained from the Spearman correlation test based on SPSS 24.0 analysis obtained a significance value of 0.768 (p>0.05) for the relationship between the level of empathy and the level of burnout. Conclusion: There is no relationship between the level of empathy and the level of burnout in students of the Medical Study Program at Tanjungpura University class 2019.
